There are two types of registrations available under the MyHEALTHCARDTM app, including for both the HEALTHCIRCLETM and SESAMASIHATTM programmes:

1. Individual Registration

This covers the individual, dependent and family account types:

Individual

  • This is a single user for the account and has access to his or her own Patient Module and all the functions provided
  • All individual accounts will require consent from the account holder to allow for the use and access of the screening data as well as all other legal requirements.

Dependent

  • This is an account where an individual set ups for his or her single/one (1) family member who is above 55-years old (Adult) or below 18-years old (Child) who is not able to manage their screening and also have access to the application
  • Here, the main account user (who may or may not have their own individual user account) will be able to manage and control all the functions of his dependent’s Patient Module and all the functions provided
  • This account is only for a single dependent, and if there is more than one (1) dependent, multiple dependent accounts will have to be opened
  • In this type of account, registration will include the requirement of a pre-signed consent form provided by the dependent (and clearly establishing the relationship with the individual main account user) with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

Family

  • This is an account where an individual set ups for his or her direct family members (spouse and up to four children below the age of 18) 
  • All user accounts will require consent from each account holder/user to allow for the use and access of the screening data as well as all other legal requirements. Those below 18-years old will require parental approval/consent.
  • Here, the main account user (who will not have their own individual user account) will be able to jointly with his spouse (also have a main user account with full access as well) to manage and control all the functions of the Patient Module for themselves and their registered children and all the functions provided.
  • The main account users can add additional accounts for other family members including extended family members by adding (purchasing) additional accounts to the main account. All added adult accounts will have their own full access and their results shared under the family account, while additional children will be controlled by the main account users.
  • In this type of account for those added as well as those below the age of 18, registration will include the requirement of a pre-signed consent form provided by the additional users (family) including those below 18-years of age with either a parent or guardian consent with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

2. Corporate or Community Registration

The Corporate / Community User is provided to companies, institutions or any organizations that are registered with Suruhanjaya Syarikat Malaysia (SSM), Suruhanjaya Koperasi Malaysia (SKM), Jabatan Pendaftaran Pertubuhan Malaysia (ROS) or any government ministries or agencies. A corporate user also includes legally appointed public servants or officials (with all the required qualifying documents and consents as detailed below) including members of Parliament (MP), State Representatives, Village Head or Leader, or any individual that has been given the legal authority. For this, it includes:

 

  • This is an account where a nominated representative (main account user) of the entity (companies, institutions or any registered organization) setups and provisions the screening package for its staff or members within the entity. By requirement, any nominated individual will require a board resolution that is fully certified by the company secretary to nominate the company individual to act as the nominated main user account as well as all company statutory documents for approval before the account is fully activated.
  • All user accounts will require consent from each account holder/user to allow for the access of the screening data (non-identifiable data) as well as all other legal requirements. Corporate / Community main account users are not able to view the individual test results but will be able to see a summary of the key findings of the screening, including trends for certain health conditions.
  • Here, the main account user will be able to set up the staff account and decide on the packages and screening tests taken by each staff member, with the individual staff able to manage and control all the functions of the Patient Module for themselves and all the functions provided.
  • The main account users can add additional accounts for other additional staff on top of the package number of available accounts by adding (purchasing) additional accounts to the main account. All added adult accounts will have their own full access and their results shared under the entity account.
  • In this type of account for additional users, registration will include the requirement of a pre-signed consent form provided by the additional users with the authority given to the nominated representative (main account user) to manage the application and the non-identifiable data that is stored based on the tests taken. 
  • Special approvals and conditions apply to circumstances where the individual user is not able to manage the screening/application including those who are immobilized or handicapped where consent is required from a family member or legal guardian that will allow a third-party individual to manage for the individual (for example in the setting of an aged facility).
