The MYHEALTHCARD™ application allows people to undertake various health screening and management packages and have their test results (current and past) and other records uploaded and viewed on a dynamic dashboard with various health panels. This proactive platform allows them to be updated with their state of health and at the same time, engage with their doctor to seek further consultations and manage their prescribed as part of this community-based health screening and clinical management platform.

The new MYHEALTHCARD™ mobile application (iOS/Android) will be available as a free download at the Apple App Store and Google Paly Store from April 2022. You will need to have an active MYHEALTHCARD™ account to download the app and link your web-app and health database or register for one from the app itself.
This new community web-app application is a free extension-app to all MYHEALTHCARDTM users. It will allow various communities to manage their members screening and health status as well as view the prevalence of the NCDs within their own communities.
KEY FEATURES
Three types of MYHEALTHCARD™ Networks account:
- Family – open up an account for your extended family with no limits on the number of family members who can be part of the family community
- Corporate – companies are able to open an account for all its staff and manage the company and staff screenings
- Community – general communities (including mosques, churches, temples, old folks home or universities/schools) where all members can screen together and be aware of the prevalence of key NCDs within the community
* Access and sharing of key summary of prevalence of NCDs will require consent from every individual within the community.

The patient data and records stores in the MYHEALTHCARD™ application will be accessible to the MYWAYDIABETES™ web and mobile application (iOS/Android) for users who take up any of the SESAMA SIHATTM or HEALTH CIRCLETM health management package.

MyWay Diabetes™ is the new patient-facing platform evolved from the Scottish national My Diabetes My Way platform. Since 2017, MyWay Digital Health has supported commissioning and deployment around the UK (North West London (Know Diabetes), Manchester (Diabetes My Way), Somerset, Lancashire and South Cumbria, Cheshire and Merseyside) and internationally. My Diabetes My Way contains i) a public-facing localised website with over 250 education resources, ii) QISMET accredited structured e-learning courses for type 1, type 2, gestational diabetes and diabetes prevention, iii) a health record (data) access patient portal

EXPLAINING THE OVERALL SCREENING PROCES
We provide our screening and management packages via our affiliate clinics and hospitals nationwide. You can also undertake screening at any of out partner screening centres. Click here for a list of our screening partners and location.
WHAT DO WE SCREEN FOR

Screening is easy.
- Book for a screening session under the ‘Book a Test’ or via the ‘Marketplace’ tab on the web application main menu bar
- Choose the screening package, an available screening location and a date/time. You can also choose if you wish to have a home screening for a fee.
- Check to understand the terms and conditions for the screening.
- Once confirmed to proceed, you will be forwarded to the payment page and make the payment for the session.
- Once you have confirmed and submitted the booking, an SMS and email notification will be sent to you.
- You will be further notified by SMS/Email three (3) days before the screening date to confirm. You can cancel for a refund or reschedule a new date no later than three (3) days before the screening date.
- You will have to prepare for the night before for the screening. Click the ‘Do’s and Don’t’s Before Screening’ to view the information of how to prepare before a screening test: English version, BM version, Mandarin version.
- On the day of screening, show up and show your QR Code and your National IC card (NRIC) to the screening technician. They will take your records, vital signs and will proceed with the withdrawal of your blood sample from your arm and you will then be required to take a urine sample.
- And all is done within 10-minutes.
- Results will be uploaded within 24-hours directly into the app and you will receive an SMS/Email notification that the results have been uploaded.
